PCSC Volunteer Background Check Forms
Please see below for a link for the expanded background check, and DCS request. Once you click on the link below, it will ask you for your name, email, and position. For the position section, please select ” EMPLOYMENT NON CERTIFIED.”
Once you complete the initial portion of the expanded background check, you will receive an additional email from safe hiring with an invitation to complete the Indiana DCS portion of the check.
(Please note if you have ever lived out of state, and are prompted to complete any additional paperwork, you do not need to do that. Indiana DCS is all you need to complete.)
